Change requests are inevitable on a project, and anyone from the client to a team member can make them at any time, therefore, an integrative project change request form is required. While satisfying the need to track and manage all those requests, a change request form also helps the requestor (of the change) to specify what exactly they would like to change in (or add to or remove from) the project scope and why. Additionally, the form allows the project manager to mention the impact of the change, which in turn helps the Change Control Board decide whether to approve the change request. Background Information

While an  integrative project change request form helps the requestor (of the change) state and justify the request, it also clarifies to the project manager, the impact this change will have on schedule, cost, risk, etc. Standardizing the change request process with a form is in the best interest of all stakeholders, both external and internal.
